A Patient's Guide to Male Infertility
(Assessment,
Diagnosis, and Treatment. By Stephen F. Shaban, M.D. Clinical Assistant
Professor Department of Surgery, Division of Urology University of North
Carolina School of Medicine Chapel Hill, NC. ``Approximately 15% of couples
attempting their first pregnancy meet with failure. Most authorities define
these patients as primarily infertile if they have been unable to achieve
a pregnancy after one year of unprotected intercourse. Conception normally
is achieved within twelve months in 80-85% of couples who use no contraceptive
measures, and persons presenting after this time should therefore be regarded
as possibly infertile and should be e valuated. Data available over the
past twenty years reveal that in approximately 30% of cases pathology is
found in the man alone, and in another 20% both the man and woman are abnormal.
Therefore, the male factor is at least partly responsible in about 50%
of infertile couples.``

Impotence,
difficulty
having an erection, is a common complaint among men. Difficulty
having an erection can occur occasionally or may be a longstanding problem
and the source of considerable tension in a relationship. Psychological
factors including anxiety, fatigue and stress can frequently cause impotence.
Medications used to treat hypertension, sleeping pills, tricyclic antidepressants
(Elavil, Pamelor and others) and cimetidine (Tagamet) can interfere with
the ability to have an erection. Physical problems including nerve damage,
diabetes, abnormalities in the circulatory system (which transports blood)
as well as hormonal abnormalities such as a low testosterone level may
result in impotence. The treatment for impotence will depend on the cause;
counseling may be helpful if psychological factors are responsible. If
a medication is causing impotence, simply changing to another medicine
may be all that is necessary. Testosterone can be given as either a shot
or through a patch which is worn on the skin, if there is a low level of
testosterone. A type of suction device can be placed on the penis prior
to intercourse; this causes blood to go into the penis, resulting in an
erection adequate for intercourse. The injection into the penis of a medicine
called Caverject can cause an erection which generally lasts an hour. Other
treatments include a pill called yohimbine (Yocon) and a surgical implant
in the penis(penile implant). Because difficulty having an erection may
be a source of embarrassment, many men will not discuss this issue with
others; however raising this issue with a physician may lead to improvement
due to one of the treatments described above. (MedicalInfo)
