  • About Dermatitis Herpetiformis  `` It is a very itchy skin rash which usually occurs on the elbows, buttocks and knees, although any area of the skin may be affected. The rash consists of red raised patches and small blisters. There are many other rashes which affect the knees and elbows and the diagnosis of dermatitis herpetiformis is not always easy and should only be made after examination and tests carried out by a specialist in skin diseases.`` The Coeliac Society, PO Box 220, High Wycombe, Bucks, HP11 2HY

Just how these grains, which are harmless to most people, damage the intestinal mucosa in people with celiac disease remains uncertain..... ``The North American Society for Pediatric Gastroenterology and Nutrition.

  • Dermatitis Herpetiformis ``The first mention of Dermatitis Herpetiformis (DH) in the literature was in 1884 in Dhring. The connection to wheat was made in Dreke, Holland in 1941. It is an uncommon, but not rare, disease that affects males twice as often as females. It is found in 10% of first degree relatives. There is a genetic association; 90% of DH patients have HLA-B8 vs. only 15% of the general population. HLA-DRw4 and HLA-DQw2 are also associated with some DH patients.`` Canadian Celiac Association's May 1998 Conference on DH

  • DERMATITIS HERPETIFORMIS ``This is a rare disorder which is associated  with celiac disease and will respond to a gluten-free diet. It is very pruritic and is characterized by urticaria and vesicles which are grouped together ("herpetiform") bilaterally and symmetrically, involving the elbows, knees, upper back, and buttocks. Histologically, the early lesions show a fibrin and neutrophil accumulation at the tips of dermal papillae, which form small microabscesses and continue to form blisters as the dermoepidermal junction  separates. The blister fluid contains more neutrophils than eosinophils, but both often  are present. By direct immunofluorescence, this disease has granular deposits of IgA selectively at the tips of dermal papillae.`` . Cornell University Medical College

The eruption tends to be persistent: only 10-15% of patients have spontaneous    remission over a 25-year study period. The disease is characterized by the presence of IgA deposits in the upper dermis skin and the diagnosis should not be made in the absence of these deposits. The HLA-B8 gene is found in 80-90% of patients with gluten enteropathy and dermatitis herpetiformis.``
